Chemistry

1. Troponin I cTroponin I interpretation in suspected Acute Coronary Syndrome (ACS):
  • Rule-in ACS: Value >600 ng/L (WHO rule-in level)
  • Rule-out ACS: Value ≤38 ng/L (females) ≤53 ng/L (males)
  • (99th percentile level), if more than 6hrs after onset of chest pain
  • Less than 6hrs of chest pain and values < rule-in level requires repeat sampling 3 hours later to demonstrate a rise in value.
Note that Troponin results obtained from different instruments have different cut-off values and are NOT comparable. This result was obtained using a Siemens Atellica instrument.
2. Serum Folate Serum folate: Deficient levels < 7.6 nmol/L
3. B-hCG Beta-hCG values during pregnancy (when performed on a Siemens Atellica analyzer):
  • < 10 IU/L: Negative
  • 10 - 25 IU/L: Indeterminate
  • > 25 IU/L: Positive
Approximate Gestational Age and HCG range:
  • 2 - 4 weeks: 39 - 8,388
  • 5 - 6 weeks: 861 - 88,769
  • 6 - 8 weeks: 8,636 - 218,085
  • 8 - 10 weeks: 18,700 - 244,467
  • 10 - 12 weeks: 23,143 - 181,899
  • 13 - 27 weeks: 6,303 - 97,171
  • 28 - 40 weeks: 4,360 - 74,883
4. Alpha Fetoprotein AFP values in pregnancy (when performed on a Siemens Atellica analyzer):
  • 15 weeks: Median 31.3 ug/L, MoM x 2 62.6, MoM x 2.5 78.3
  • 16 weeks: Median 36.3 ug/L, MoM x 2 72.6, MoM x 2.5 90.8
  • 17 weeks: Median 42.0 ug/L, MoM x 2 84.0, MoM x 2.5 105.0
  • 18 weeks: Median 48.7 ug/L, MoM x 2 97.4, MoM x 2.5 121.8
  • 19 weeks: Median 56.5 ug/L, MoM x 2 113.0, MoM x 2.5 141.3
  • 20 weeks: Median 65.4 ug/L, MoM x 2 130.8, MoM x 2.5 163.5
(MoM = Multiples of the median) Gestational age-related reference ranges for AFP are used to aid the diagnosis of Neural Tube Defect (NTD) and Down's syndrome (in combination with multiple serum markers eg hCG and estriol). Any AFP value >2 times the median should be investigated by ultrasound examination to exclude foetal malformations as well as multiple gestation, foetal death and incorrectly estimated gestational age.
